Travoprost (Travatan Z) Risks, Warnings, and Complications


Travoprost ophthalmic solution can cause or worsen medical problems in some people. Talk to your doctor about other options to treat open-angle glaucoma or ocular hypertension if you are at increased risk of complications from this medication. Some of the risks of travoprost use include:

  • PIGMENTATION: Travoprost (Travatan Z) drops can cause changes in pigmented (colored) tissues such as the iris of the eye as well as periorbital tissues such as the eyelid and eyelashes. The changes in iris color are usually permanent, but pigmentation in the periorbital tissue and eyelash changes may be reversible. Experts do not know the long-term effects of these pigmentation changes.

    The changes in iris color in the treated eye may not appear for several months or even years after starting travoprost use. The pigmentation usually consists of the iris becoming more brown over time due to an increased amount of melanin pigment in the melanocytes. 

  • EYELASH CHANGES: Travoprost can cause the eyelashes to gradually become longer, thicker, darker, and increase in number. These eyelash changes are typically reversible and go away when travoprost treatment is stopped.

  • EYE INFLAMMATION: Travoprost can cause worsening of eye inflammation (uveitis) and should be used with caution in people who have active intraocular inflammation.

  • MACULAR EDEMA: Travoprost can cause macular edema (fluid buildup in the central part of the retina called the macula). This medication should be used with caution in people with certain eye problems and known risk factors for macular edema.

  • INFLAMMATORY, NEOVASCULAR, OR ANGLE-CLOSURE GLAUCOMA: Travoprost is used to treat open-angle glaucoma. Its use has not been evaluated for safety or efficacy in other types of glaucoma, such as inflammatory, neovascular, or angle-closure glaucoma.

  • BACTERIAL KERATITIS: There are reports of eye infection (bacterial keratitis) in people who used multiple-dose containers of eye drops due to contamination. Additional risk factors for eye infection include corneal disease or disruption of the ocular epithelial surface (eye surface). Your health care professional will tell you how to use travoprost eye drop correctly. Follow these instructions carefully to prevent contamination and eye infection.

  • CONTACT LENSES: If you wear contacts, remove them before instilling travoprost and reinsert them 15 minutes later.


Precautions Before Starting Travoprost (Travatan Z)

Tell your doctor if you have had allergic reactions to travoprost, any of the inactive ingredients in travoprost eye drops, or any other drugs in the past. Your pharmacy can give you a list of ingredients.

Give your doctor and pharmacist a complete list of your medications, including prescription drugs, nonprescription medicines, vitamins, supplements, and herbal products. This can help avoid possible drug interactions between travoprost ophthalmic and your other medicines.

Tell your doctor if you have pre-existing medical conditions. Travoprost may not be safe for people with certain health conditions. You may not be a candidate for travoprost implant if you have cornea problems.

Tell your doctor if you are pregnant, could be pregnant, are planning a pregnancy, or nursing an infant with breast milk.

Precautions During Use of Travoprost (Travatan Z)

Keep all your healthcare appointments while taking travoprost.

If you are using other eye drops, including other eye drops to lower pressure in the eye, make sure you apply them at least 5 minutes away from travoprost.

If you wear soft contact lenses, remove them before using travoprost. Wait at least 15 minutes before putting your contact lenses back in.

If you have an eye injury or eye surgery, check with your healthcare professional before continuing to use the same container of travoprost.

Tell your healthcare providers you have a travoprost implant (iDose TR) before any magnetic resonance imaging (MRI) study.

Tell your healthcare professional if you become pregnant while on travoprost. 

Let your doctors know that you are using travoprost ophthalmic before any surgery, including dental procedures.

Travoprost can cause temporary blurring of vision. Do not drive, operate machinery, or do other activities that require clear vision until you can see clearly.

Travoprost (Travatan Z) Drug Interactions

Concomitant administration of travoprost eye drops with certain medicines can affect how the medicines work. Possible drug interactions between travoprost and other medicines can also increase the risk of severe adverse events. Your doctor may change the dose of your medications, switch you to a different medication, or monitor you carefully for side effects if there are known interactions between travoprost and your other medicines.

Give your doctor or pharmacist a complete list of your medications, including other prescription drugs, non-prescription medicines, dietary supplements, and herbal remedies. Also, tell your healthcare provider if you smoke, drink alcohol, or use recreational drugs because some of these substances can cause serious health complications when used with prescription and over-the-counter medicines.
