The generic drug semaglutide, which is the active ingredient in Rybelsus, is also available as a once-weekly injectable medicine for people with diabetes under the brand name Ozempic.

Semaglutide has also been FDA-approved for chronic weight management in overweight and obese adults. The semaglutide injection is sold under the trade name Wegovy for weight management.

Rybelsus Mechanism of Action

Rybelsus helps to improve blood sugar by causing the pancreas to release more insulin hormone and the liver to decrease glucagon secretion. These hormones play a key role in controlling blood sugar levels in the body. Rybelsus (semaglutide) also slows the movement of food through the stomach and can increase feelings of fullness, leading to decreased appetite and weight loss.

Rybelsus Doses

Rybelsus comes as 3 mg, 7 mg, and 14 mg tablets. Other dosage forms of semaglutide include Ozempic weekly semaglutide injection for type 2 diabetes and Wegovy weekly semaglutide injection for weight management.

The usual starting dose of Rybelsus (semaglutide oral) is 3 mg once daily for 30 days. The 3-mg dose does not help with glycemic control (blood sugar control) and is for treatment initiation only (to give your body time to adjust to the medicine). After 30 days, the dose of Rybelsus is increased to 7 mg once daily. After another 30 days, the Rybelsus dose can be increased to 14 mg once daily, if needed.

Wegovy helps with weight loss when taken along with a lower-calorie diet and increased physical activity. It can also reduce the risk of major adverse cardiovascular events such as heart attack, stroke, and death in overweight and obese individuals with cardiovascular disease.

Wegovy Mechanism of Action

Wegovy (semaglutide) is almost identical to a natural hormone in the human body called glucagon-like-peptide-1 (GLP-1). GLP-1 helps to regulate appetite and food intake. Wegovy (semaglutide) binds to and activates GLP-1 receptors in the brain, slows movement of food through the stomach, suppresses appetite, and thereby leads to reduced caloric intake and weight loss. It is unclear how Wegovy lowers the risk of heart attack, stroke, and cardiovascular death in people with established cardiovascular disease.

Wegovy Doses

Wegovy comes in a single-dose, pre-filled, disposable pen for subcutaneous injection (under the skin of the upper arm, abdomen, or thigh) once a week. You should take a Wegovy injection on the same day of the week every week. You can take it at any time of the day without regard to meals. 

The dosage strengths of Wegovy pens are 0.25 mg/0.5 mL, 0.5 mg/0.5 mL, 1 mg/0.5 mL, 1.7 mg/0.75 mL, and 2.4 mg/0.75 mL. The dose of Wegovy is gradually increased as follows:

  • Weeks 1 through 4: 0.25 mg once weekly
  • Weeks 5 through 8: 0.5 mg once weekly
  • Weeks 9 through 12: 1 mg once weekly
  • Weeks 13 through 16: 1.7 mg once weekly
  • Week 17 onwards: 1.7 mg or 2.4 mg once weekly (maintenance dose)

Ozempic is a once-weekly non-insulin injectable prescription medicine for adults with type 2 diabetes mellitus. It is used to control blood sugar levels, along with diet and exercise. It is also used to reduce the risk of major adverse cardiovascular events such as heart attack, stroke, and death in people with established cardiovascular disease and diabetes.

Ozempic is not used to treat people with type 1 diabetes or diabetic ketoacidosis. Also, Ozempic cannot be used instead of insulin in people who need insulin for blood sugar control.

Ozempic Mechanism of Action

Ozempic is an injectable prescription medicine available as a pre-filled single-patient-use pen. The Ozempic pen delivers 0.25 mg, 0.5 mg, or 1 mg of semaglutide per injection. The pen is designed for subcutaneous injection (injection under the skin’s surface) and is available as 2 mg/1.5 mL and 4 mg/3 mL.

The usual starting dose of Ozempic is 0.25 mg once a week. After 4 weeks, your doctor may increase the dose to 0.5 mg once a week. After an additional 4 weeks, your Ozempic dose may be increased to 1 mg once a week, depending on blood sugar control. If additional glycemic control is required after at least 4 weeks on the 1 mg dosage, your dosage may be increased to 2 mg once weekly. The maximum dose of Ozempic is 2 mg once a week.

How To Use Rybelsus

Rybelsus comes as an oral tablet to take by mouth. You should take it first thing in the morning on an empty stomach, at least 30 minutes before eating or drinking anything or taking other oral medicines. Take a Rybelsus (semaglutide) tablet with no more than 4 ounces (120 mL) of plain water. Taking medications or eating or drinking within 30 minutes can affect Rybelsus by reducing its absorption and making it less effective. 

Eat something 30 minutes after taking your Rybelsus (semaglutide) dose. Waiting more than 30 minutes to eat or drink after taking Rybelsus can lead to increased absorption of Rybelsus. Too much Rybelsus in your body can put you at risk of low blood sugar.

Do not cut, chew, or crush Rybelsus (semaglutide) tablets. If you are on the 14-mg dose of Rybelsus, do not take two of the 7-mg tablets to make a 14-mg dose.

Read the prescription label carefully and take Rybelsus exactly as prescribed. Do not change the dose or dosing frequency without consulting your healthcare provider.

Your doctor will start you on a low dose of Rybelsus and increase the dose every 4 weeks until you are on a maintenance dose that controls your blood sugar levels. 

You should know that Rybelsus works alongside a calorie-restricted diet and increased physical activity to help control type 2 diabetes. This medication is not a cure for diabetes. Continue taking Rybelsus even if you feel well and your blood sugar levels are well controlled. Do not stop taking this medication without talking to your doctor.

Missed dose: If you miss a dose of Rybelsus, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take a double dose or extra medicine to make up for a missed dose.

Storage: Store Rybelsus (semaglutide) at room temperature (between 15-30° C or 59-86° F) away from moisture, heat, and direct light in its original container, tightly closed. Do not store medications in the bathroom where they can be exposed to moisture and heat. Keep all medications safely out of reach of children and pets.

Unused medicine: Unused Rybelsus should be disposed of in a way that other people, children, and pets cannot get to them. Do not throw Rybelsus in the trash or flush it down the toilet. How To Use Wegovy

Wegovy (semaglutide) comes as a liquid solution in a prefilled single-use dosing pen for injection subcutaneously (under the skin). It can be injected at any time of the day, with or without food. However, you should inject Wegovy on the same day of the week every week. If you want to change the day of the week that you inject Wegovy, you can do so as long as at least 48 hours (2 days) have passed since your last injection. 

Read the prescription label carefully and follow your doctor’s dosing instructions. Use Wegovy exactly as prescribed. Do not change the dose or dosing frequency without your doctor’s approval.

Your doctor will likely start you on a low dose of Wegovy and increase the dose slowly every 4 weeks until you reach a maintenance dose. 

You can inject Wegovy in your upper arm, stomach, or thigh areas. Rotate (change) the injection site every week. You may inject Wegovy (semaglutide) and insulin in the same general area of your body, but not immediately next to each other. 

Wegovy can help you lose excess weight when used with a reduced-calorie diet and increased physical activity, but it is not a cure for either obesity or being overweight. Continue taking Wegovy as long as your doctor advises. Do not stop treatment without talking to your healthcare provider.

Your doctor or pharmacist will show you how to correctly inject Wegovy. Make sure you understand these instructions. Ask your healthcare providers if you’re unclear about any part of the process. 

Wegovy pens that are stored in the refrigerator should be left to warm to room temperature before the injection. Check the injection solution in the Wegovy pen before injecting it. The solution should be clear and colorless without any visible particles. Do not inject Wegovy if the solution appears thickened, cloudy, colored, or has visible solid particles. Also, do not use Wegovy pens that are past the expiration date.

Never reuse a Wegovy pen or share it with others. This is a single-dose disposable pen and should be disposed of in a puncture-resistant container as explained by your doctor and pharmacist.

Missed dose: If you miss a dose of Wegovy, take it as soon as you remember as long as your next dose is at least 48 hours (2 days) away. If there are less than 48 hours left for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take a double dose or extra medicine to make up for a missed dose. If you miss more than two doses of Wegovy, call your doctor. You may need to go back to a lower dose.

Storage: Store Wegovy pens in the refrigerator (36°F to 46°F or 2°C to 8°C). You can also store the pens at room temperature (46°F to 86°F or 8°C to 30°C) in the original carton, as long as the cap hasn’t been opened, for up to 28 days. Do not freeze Wegovy. Do not use Wegovy pens that were frozen and thawed.

How To Use Ozempic

Ozempic comes as a liquid solution in a pre-filled pen for subcutaneous injection under the skin. This medication is typically injected once a week in the abdomen, thigh, or upper arm. You can administer the Ozempic injection at any time of day, regardless of meal times. However, you should inject Ozempic on the same day of the week every week. If you wish to change the day of the week you inject Ozempic, make sure it has been at least 48 hours (2 days) since your last dose.

You can inject Ozempic in your stomach area, thigh, or upper arm. Rotate (change) the injection site every week. You can inject insulin and Ozempic in the same area, but do not give yourself the injections right next to each other. 

If you have stored an Ozempic pen in the refrigerator, allow it to warm to room temperature before using it.

Make sure the Ozempic injection solution is clear, colorless, and free of particles before injecting it. Do not use the medication if it is cloudy, colored, or contains floating particles. Do not use an Ozempic injection pen that is past its expiration date.

However, if more than 5 days have passed since the missed dose, skip it and take the next dose as scheduled. Do not take two doses together to make up for a missed dose.

Storage: 

  • You should store unused Ozempic pens in the refrigerator (36-46°F or 2-8°C). Avoid placing the pens near the cooling element of the refrigerator. 
  • Once an Ozempic pen is in use, you can store it in the refrigerator or at room temperature (59-86°F or 15-30°C).
  • Never freeze Ozempic pens. Do not use Ozempic if the medicine has been frozen. 
  • While traveling, you can store your Ozempic pens at room temperature but do not keep them in the glove compartment or other hot places in a car. 
  • Note down the date when you first start using an Ozempic pen. Discard the pen after 56 days, even if some medicine is left in the pen.

Unused medicine: Throw away any expired or unused Ozempic. Do not flush the medicine down the toilet. Rybelsus Warnings (Brand)


Rybelsus Risks, Warnings, and Complications


Rybelsus can cause or worsen health problems, especially in certain high-risk patients. Some of the risks of Rybelsus therapy include:

  • RISK OF THYROID TUMORS: Animal studies have indicated that Rybelsus (semaglutide) can cause an increase in the risk of thyroid cancer called thyroid C-cell tumors, including medullary thyroid carcinoma. This risk increases at higher doses and with a longer treatment duration. It is unclear if there is an increased risk of thyroid cancer related to semaglutide use in humans. However, there are reports of medullary thyroid cancer in people treated with liraglutide, which is also a GLP-1 receptor agonist. The data is insufficient, however, to establish a direct causal relationship between thyroid cancer and the use of GLP-1 receptor agonists in humans.

    People with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2) should not take Rybelsus (semaglutide). 

    You should seek immediate medical attention if you develop signs and symptoms of thyroid tumors, such as a mass in the neck, hoarseness, trouble swallowing, or shortness of breath. 

  • PANCREATITIS: Controlled clinical trials have shown that people taking Rybelsus can develop pancreatitis (inflammation of the pancreas). Tell your doctor immediately or seek emergency medical attention if you develop signs and symptoms of pancreatitis, such as ongoing severe pain in the abdomen (middle or upper stomach), which may radiate to the back, with or without vomiting. You will need to stop taking Rybelsus (semaglutide oral tablets) if you develop pancreatitis.

  • VISION PROBLEMS RELATED TO DIABETIC RETINOPATHY: Taking Rybelsus has been linked to an increased incidence of diabetic retinopathy related complications in controlled trials. This risk is higher among patients with a preexisting history of diabetic eye problems. Scientists believe that the rapid improvement in blood sugar control can cause a temporary worsening of diabetic eye complications. We do not know the long-term effects of Rybelsus-induced blood sugar control on diabetic eye complications. Tell your doctor if you have a history of diabetic retinopathy. Your provider will monitor you carefully for the progression of the condition.

  • SEVERE HYPOGLYCEMIA (LOW BLOOD SUGAR): People taking Rybelsus (semaglutide) and other diabetes medications such as insulin or insulin secretagogues (sulfonylureas) are at an increased risk of severe hypoglycemia or dangerously low blood sugar. Your doctor may adjust the dose of your other diabetes medications after starting you on Rybelsus. Talk to your doctor about how to recognize hypoglycemia (low blood sugar) and what to do if it occurs.

  • ACUTE KIDNEY INJURY: There are postmarketing reports of kidney damage and worsening kidney failure, including the need for hemodialysis, related to the use of semaglutide and other GLP-1 receptor agonists. Acute kidney injury has been reported in patients with no prior history of kidney problems. Risk factors for kidney damage include dehydration, nausea, vomiting, and diarrhea. Your health care provider will monitor you for kidney problems when you first start Rybelsus (semaglutide), after an increase in the dose, or if you develop severe gastrointestinal side effects.

  • SERIOUS ALLERGIC REACTIONS: There are reports of severe allergic reactions, including anaphylaxis (throat closing) and angioedema (swelling) in patients treated with Rybelsus (semaglutide). Stop using Rybelsus and seek emergency medical care if you develop signs and symptoms of serious allergic reactions, such as skin rash, hives, itching, swelling, or difficulty breathing. People who have previously had a serious allergic reaction to Rybelsus should not take this medicine again. Rybelsus should be used with caution in people with an allergic reaction to other GLP-1 receptor agonists.

  • GALLBLADDER PROBLEMS: There are postmarketing reports of cholelithiasis (gallstones) and cholecystitis (gallbladder inflammation) related to GLP-1 receptor agonist use.   • DIABETIC RETINOPATHY: Clinical trials have shown that adults with type 2 diabetes and BMI 27 kg/m2 or greater can develop diabetic retinopathy (eye complications).
